To stop rising rates of obesity in Missouri and to improve the health of Missourians, the Missouri Foundation for Health (MFH) invested over $20 million to support 87 Healthy & Active Communities (H&AC) projects since 2005. In 2007 and 2008, 19 Model Practice Building (MPB) grantees were funded as part of the H&AC Initiative to become promising practices and, eventually, model projects for Missouri. A joint report from CPHSS, Saint Louis University School of Public Health, and the University of Colorado, Colorado Springs, Beth-El College of Nursing and Health Sciences presents case examples of five MPB grantees that demonstrated the Model Practice components of innovation, effectiveness, and sustainability. The Model Practices highlighted in the report and lessons learned were used by MFH to guide future funding decisions.
